We are looking for experienced, dynamic Nurses to provide a proactive, personalised and multidisciplinary case management approach to frail and elderly patients in Wandsworth borough with complex long-term conditions, who are at risk of unplanned hospital admission to improve their quality of life, prevent future unplanned admissions A&E attendance, admission to hospital or long term care environments, support early discharge home if and/or when admitted, and support older people to remain at home.

Main duties of the job

The team operates 5-days a week between 9am and 5pm although there is scope for flexibility within the working hours to accommodate family friendly working patterns.

Job responsibilities

  • To provide a proactive, personalised and multidisciplinary case management approach to frail and elderly patients in Wandsworth with complex long-term conditions, who are at risk of unplanned hospital admission to improve their quality of life, prevent future unplanned admissions A&E attendance, admission to hospital or long-term care environments, support early discharge home if and/or when admitted, and support older people to remain at home.

  • To pro-actively draw on personal expertise and skills/ knowledge of all team members to improve outcomes for patients and carers.
  • To proactively monitor and anticipate the changing needs of the patients, co-ordinating their care across health and social care system.
  • To participate in the delivery of educational programmes to patients, carers and health and social care workers that promotes self-care principles.
  • o follow agreed case management pathway/flow diagram to ensure consistency of process.
