Job summary

Band 3 Rehabilitation Assistant - Tri-Borough Falls Prevention Service - Colville Health Centre

We are looking for a new Rehabilitation Assistant to join our renowned Tri-Borough Falls Prevention Service within CLCH. The service sees residents of Westminster, Kensington & Chelsea and Hammersmith & Fulham who are at risk of falls. We predominantly see clients in clinics and deliver evidence based strength and balance classes but we do still see a small number of clients in their own homes for one to one rehabilitation.

The post holder will work alongside a team of Physiotherapists, Occupational Therapists and other Rehabilitation Assistants and will be supported by their Team Lead and Falls Clinical Lead. The post holder will have a key role in helping to deliver strength and balance classes alongside therapists and will also work independently in clinic settings teaching exercises, checking memory, balance, fear of falling as well as measuring blood pressure.

There will also be opportunities to work with clients in their own homes to improve mobility, balance, function and safe use of equipment under specific direction of a therapist.

Main duties of the job

The successful candidate will be given specific training on various outcome measures and screening tools and once competent will conduct these assessments independently in community and clinic settings.

The Tri-borough Falls Prevention Service provides an excellent in-service training programme; including rehabilitation assistant specific sessions and regular clinical supervision.   • To contribute to the on-going assessment of a patient by completing the appropriate paperwork for allocated patients, recording and reporting all clinical and behavioural information to the nurse/therapist for action and planning purposes.
  • To undertake, and report back on delegated clinical activities that have previously been signed competent. E.g.: vital signs, gait/mobility training, falls prevention groups, increasing independence through activities of daily living
  • With the appropriate training the RA will plan, design and deliver higher level strength and balance groups for patients/clients with a history or high risk of falls who have been identified as suitable for group exercise by the Falls Specialist Therapists.
  • Within the remit of the higher level strength and balance group, review clients/patients exercise programmes.
  • To enable patients/carers to make most effective use of resources within the service by ensuring they receive up-to-date information sheets, responding to any questions constructively
  • To pass onto/seek assistance from other colleagues, promptly, any questions that you are unable to resolve
