Respiratory Practitioner, Home Oxygen Service Assessment and Review (HOSAR) (Physiotherapist or Nurse) - Wandsworth Respiratory Team

Band 6 Permanent (37.5 hrs per week. Full time or part time/job share considered)

The Wandsworth Respiratory team delivers a range of services within the London Borough of Wandsworth in a variety of settings. The team consists of respiratory nurses and a Health care assistant.

We are seeking a motivated, caring and dynamic Respiratory practitioner to work as a key member of our new HoSAR team. The respiratory team is entering an exciting new stage of development, expanding the services that we provide in order to improve patient outcomes within the borough. The post holder's role will be diverse and centred within the respiratory service, whilst supporting the lead for the HOSAR service.

Main duties of the job

This role involves seeing complex respiratory patients in a range of settings including various community sites and patients homes in addition to community clinics. The post holder will autonomously manage their own caseload of patients as well as provide support and cover for the other clinicians within the team. We have structured clinical supervision provided by one of the local consultants and support each other through team meetings, shared learning and reflective practise. You will receive regular 1:1 meetings with the Operational Manager to support you in your role.

As a community trust we utilise "agile working" meaning that you will often work remotely. You will therefore need to be a flexible and adaptable team member with excellent communication skills including written and verbal. Good time management, basic IT skills and willingness to learn new skills.

The team operates 5-days a week between 9am and 5pm although there is scope for flexibility within the working hours to accommodate family friendly working.

The Practitioner will work within a multidisciplinary respiratory team to deliver the Respiratory Service, specifically in relation to oxygen therapy within the CLCH South-West Division (Wandsworth)

To support the Home Oxygen Service and Review (HOS-AR) lead in the development and running of the service in Wandsworth

To support in the assessment and treatment of patients within the HOS-AR, who have highly complex physical, psychological and social needs and work as an autonomous practitioner as well as part of a team

To support the HOS-AR lead in co-ordinating and providing the community-based oxygen assessments and management clinics.

Work with the HOS-AR/Respiratory team in assessing, planning, co-ordinating, delivering and evaluating care for patients receiving home oxygen and those attending respiratory review clinics.

The post holder will spend at least 80% of the time providing patient facing or related care.

To support and provide information to service users as required i.e. GPs, Primary and secondary care professionals, voluntary organisations, patients and their careers.

To provide safe practice to all patients referred to the CLCH Wandsworth HOS-AR and Community Respiratory Service and liaise with Primary and Secondary Care Practitioners regarding the care and medical management of patients with respiratory problems.
