Job summary

An exciting part time (15 hours a week) opportunity has arisen for a Band 6 Tissue Viability Specialist Nurse to join CLCH NHS Trust's community tissue viability team in West Hertfordshire. We require a highly motivated and skilled RGN with a BSc Level CPD in Tissue Viability related discipline who is confident in working across professional boundaries. You will need to flexible, innovative and a good team player with the ability to work independently within a community setting. Please note that the majority of patient interventions are performed in patients' homes, so it is essential that you hold a full driving licence.

You will be joining a friendly supportive team and a large NHS community Trust where professional guidance is readily available and personal and professional development is fully supported.

Main duties of the job

Advise first-line HCP's regarding all aspects of Tissue Viability best practice in relation to their caseloads. Holistically assesses and manages patients with all types of tissue viability needs and plans care in conjunction with patients' caseload holders.

Assesses and plans care for complex patients, including skin care, exercise, compression therapy and advanced wound therapies in order to support care close to home and avoid hospital admission where possible.

Manages the prevention and treatment of pressure ulceration and related skin conditions, including exacerbations and develops specialist knowledge to be able to advise staff, patients and carers. Assists in the delivery of an effective and comprehensive advisory Tissue Viability service, working closely with the leg ulcer and lymphoedema services.

With the support of lead practitioner, delivers training, formally or informally, on all aspects of tissue viability management to patients, carers and to all staff groups in the multidisciplinary team.
