CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

CLCH Hertfordshire Division covers the 4 localities of West Hertfordshire, delivering care across the population. We are setting up a new team to deliver care for residential homes across three of our four geographical localities - Hertsmere, St Albans and Harpenden and Watford - to improve continuity and enable the residential homes and staff within CLCH to feel well supported and able to deliver excellent care.

Main duties of the job

This idea has come from clinicians delivering face to face contact with patients in the Covid pandemic. They have demonstrated the enhanced level of support we can offer homes through training and collaborative working, and they have built close relationships with the homes as a result.

This has achieved reduction of pressure ulceration, effective treatment of leg ulceration, improvements in continence care, palliative care and management of long term conditions.

The outbreak team healed one patient who had had daily dressings on their legs for over a year in under four weeks, and the quality of life the patient now has is vastly improved from before.

  • To provide leadership and management to a team of community nurses and other staff covering the four localities across Herts Valleys.
  • To be responsible for the teams assessment, planning, implementation and evaluation of individualised nursing care to patients living in a residential home. The service is provided seven days per week 8pm-8am

  • Registered Nurse with current UK NMC registration
  • Practice Supervisor/Assessor or equivalent
  • Specialist Nursing Practice (District Nursing) Degree or will undertake training
  • Nurse Prescribing or will undertake training
  • Evidence of personal development

Desirable

  • First Level Degree
  • Independent Prescriber
  • Relevant courses in Long Term Conditions Management
  • Course of Top to Toe examination/ diagnosis experience
