Job summary

An exciting opportunity has arisen for anenthusiastic band 4 Rehabilitation Assistant to join our MDT team at Furness rehab ward, Willesden Centre for Health, and Care.

Furness ward is an In-Patient General Rehabilitation unit-based Willesden Centre for Health and Care, NW10 3RY.

You will be part of a Multi-disciplinary Team (Medical, AHP and Nursing) in the planned patient centred rehabilitation and facilitating smooth discharge flow and safe discharge of patients coming from acute hospitals to a bedded setting within a community environment. As part of the role, you will be allocated varied caseload and providingpatientcentred careat Aster unit with the support of the MDT.

Applicants will provide a mixture of general and specific care to patients and will be proficient and competent to work at a high level of competency. They will work alongside a Doctor, Registered Nurses, HCA, and Allied Health Professionals.

The post holder will be expected to have a Foundation degree in Health and Social care or equivalent experience or qualifications. They will also become proficient and competent in their area of practice by undertaking competency assessments specific to the area of clinical practice. The post holder will be supported and supervised by a registered Allied health professional. The post holder will also support the Aster Unit to deliver against key performance indicators and maintain quality of the service.

The Rehabilitation Assistant will provide a mixture of general and specific care to a group of patients and will be proficient and competent to work at a high level of competency. They will work alongside a registered nurse, general practitioner and/or allied health professionals. The Rehabilitation Assistant will work in a variety of settings including community rehabilitation unit and patient/carers own homes.

The post holder will be expected to have a Foundation degree in Health and Social care or equivalent experience or qualifications. They will also become proficient and competent in their area of practice by undertaking competency assessments specific to the area of clinical practice. The post holder will be supported and supervised by an Allied health professional.

The post holder will also support Outreach Rehabilitation Team to facilitate smooth transition of patient discharge and to maintain quality of the service.
