An exciting opportunity has arisen to recruit a PCN Clinical Leads for Inclusive Health Primary Care Network to enhance the leadership structure across health, social care and the voluntary sector in the West London footprint.

Main duties of the job

To be responsible for the clinical, strategic and operational management of the community Integrated Care Team (ICT) across one of the five Primary Care Networks (PCN) in WL CCG with the primary aim of improving integration of services across health, social care and the voluntary sector.

The post holder will work collaboratively with system partners in the development of care pathways to ensure that services are equitable and effective and local and national standards of practice are maintained.

The successful candidate must be comfortable managing stakeholders with competing priorities and have a proven track record delivering service improvement using QI methodology. They will work closely with the clinical director of the PCN whilst coordinating delivery and development of the integrated care service in the community.

  • Current clinical registration (NMC/HPC)
  • Master's degree or equivalent level combination of post-graduate study and further training and experience.
  • Evidence of continuing professional education

Desirable

  • Qualification in Healthcare Leadership
  • Qualification in Quality Improvement methodology
