Job summary

CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

This vacancy is for a rapid response specialist nurse in our Unplanned Care team. The unplanned care team provides care across three pathways delivered over 7-days a week 8am to 10pm - Rapid Response (RR), Early Intervention Vehicle (EIV) and Discharge to Assess (DTA). All pathways are delivered in a patient's usual home environment. The overarching aim of the service is to help ease the current pressures in ED and acute hospitals. It is also to provide a safe and timely discharge from hospital and community bedded units for patients requiring urgent therapy input intheir homes.

  • RR focuses on preventing avoidable hospital admissions for patients with acute ill-health conditions by providing holistic assessment and treatments. This could be for up to 5 days.
  • EIV is a joint pathway, delivered with Herts County Council delivering crisis management to patients offering them a holistic assessment - physical, functional and social all in one assessment.
  • DTA pathway aims to support the discharge of patients from acute orcommunity hospital wards to assess and manage rehabilitation/therapy needs, at the point that the patient is medically well enough to go home.

Main duties of the job

We are looking for a nurse or paramedic with acute assessment skills to join our multi-disciplinary team of Paramedics, Nurses, Health and Rehab Assistants, Occupational Therapists, Physiotherapists, Administrators and a new addition into the team - General Practitioners.

Candidates should be experienced clinicians with sound clinical knowledge and proven experience in emergency or community clinical environments. These skills are necessary to adequately assess urgent cases in the community. You will need to be adaptable to operate in a fast-moving environment and possess the clinical skills and ability to work autonomously in a larger team.

Candidates should have excellent communication skills; and the ability to provide care as determined by clinical assessments is essential. The ability to keep accurate clinical records and have good clerical/ administrative skills is essential.

Continual personal development is very much encouraged and funding for relevant training will always be considered. Modules such as the non-medical prescribing and advanced assessment are popular choices within our team.
