Job summary

We have an exciting opportunity for a Band 4 Diabetes Primary Care Network (PCN) care and education Coordinator, to join our friendly team within Brent Community Diabetes Service. The PCN care and education coordinator plays an important role within the PCN by adopting a flexible approach to patient care and service delivery. The aim is to help patient to improve their quality of life.

The post holder will be responsible for the coordination, planning, implementation, and evaluation of the diabetes education as well as providing clinical support to patients. The postholder will be working collaboratively with GPs within Brent PCN and the Community Diabetes Specialist Service.

The post holder will take responsibility for booking clinics, processing new referrals, updating waiting lists and communicating appointment related information to patients/carers. And is expected to be able to act on their own initiative in dealing with day-to-day issues and the screening of correspondence, both written and verbal.

Main duties of the job

PCN care and education coordinator plays an important role within the PCN by adopting a flexible approach to patient care and service delivery. The aim is to help patient to improve their quality of life.

The post holder will be responsible for the coordination, planning, implementation, and evaluation of the diabetes education as well as providing clinical support to the patients.

The postholder will be working collaboratively with GPs within Brent PCN and the Community Diabetes Specialist Service. And from time to time, may be asked to work across different sites across the division
