Job summary
HVCCG are seeking to appoint a Palliative and End of Life Commissioning/Contracting Manager. This post is a split post which will provide Palliative and End of Life commissioning support to the west Hertfordshire ICP sitting within the HVCCG Planned Care Team and contracting support for the End of Life providers within Central London Community NHS Healthcare Trust (CLCH).
CLCH Contracting Support
In October 2019 CLCH were awarded the contract for Adult Community Services. End of Life Services are one of the commissioned services and CLCH is responsible for the delivery of End of Life Services in conjunction with Hospice of St Francis, Peace Hospice, Rennie Grove Hospice Care and Marie Curie
HVCCG Commissioning Support
This element of the role will provide commissioning support for HVCCG across the ICS and ICP to bring together all threads of palliative and end of life care. This will comprise developing and commissioning palliative and end of life care services locally to deliver the transformation required in line with the ICS End of Life Strategy.
Main duties of the job
CLCH Contracting Support
The role will include leading on the service transformation of palliative and end of life care services in line with contractual Service Development Improvement Plans (SDIP) and any ICS Service Transformations for Palliative and End of Life Care which may need mobilising and operationalising with key partners.
This role will also support CLCH to fulfil their contract obligations on End of Life Care to the CCG but also ensure that service transformations and new end of life services are managed effectively with the end of life partners.
HVCCG Commissioning Support
The post holder will be an active member of the End of Life ICS forums but will also work across the ICP to understand and identify any operational issues and service developments pertaining to palliative and end of life care with our local providers.
The role will lead on the local Palliative and End of Life Care Strategy/service transformation plan to ensure that this workstream is translated into commissioning and service development plans with our local providers, and support the delivery of these plans.
