Job summary

Diabetes is a key priority for us in Brent and Harrow developing a model of care for the management of patients with Diabetes. This role will support and help primary care teams to deliver this model of care and align it to the NWL diabetes guidelines.

We are looking to recruit full-time B7 Diabetes Nurse Specialists who are passionate about diabetes care. We are a multidisciplinary team consisting of consultants, dieticians, podiatrists as well as specialist nurses. The team receives referrals from many primary care services as well as local acute hospitals. We are building close working relationships with Primary Care Networks to deliver on the NWL diabetes guidelines.

Main duties of the job

The post holder will provide support to practice and other services to develop a structured approach to the management of Diabetes and work to agreed clinical protocols and guidelines ensuring comprehensive care across the diverse population of the Borough. You will be an experienced Diabetes Nurse with relevant post registration training. Ideally be an independent Nurse Prescriber or demonstrate a willingness to achieve qualification. You will have supervisory responsibility and able to guide and teach members of the clinical team. Job responsibilities

Responsible for the continuing care of patients with diabetes with sub-optimal diabetes control identified by DSN/Facilitator, GP or consultant.

To provide core aspects of diabetes care management across appropriate health care settings i.e. management of risk factors, provide basic dietary advice, treatment planning, education, screening for complications etc.

To act as an autonomous practitioner (often working in isolation), planning and implementing programmes of care for people with diabetes, their families and carers.

Influence multi-disciplinary teamwork and communication between health social and voluntary organisations to deliver coordinated seamless patient care.

Work in partnership with GP surgeries to support the development of strengths, cascade clinical skills thereby raising the standard of diabetes care to prevent micro and macro vascular complications using evidence base NICE, BHS Guidelines etc.

Work to provide diabetes services at designated GP Primary Care Localities and Intermediate Clinic

Facilitate & support practice nurse and GP resource learning, care planning and delivery of DESMOND programme not possible to train practice staff as this can only be done by DESMOND Centre.

Develop and facilitate in-house initiatives within individual practices to support the delivery of high-quality care for people with diabetes.

Develop practice level Diabetes profiling to facilitate targeted work streams and to improve patient outcomes and self-care

To help plan and participate in an annual audit plan for the Diabetes Pathway

Feedback audit data to professionals and facilitate changes needed to provide more effective and seamless care of diabetes.

Monitor gaps in service and support appropriate change.

Maintain accurate and concise records of clinical care. This may be done on GP Practice databases or on the trust clinical database (RIO).

Update colleagues in the advances in Diabetes care to ensure that practice is outcome and evidence based.

Facilitate and teach on any locally run diabetes care course.

To enable and facilitate patient decision-making in relation to the long-term management of their condition.
