Job summary

At CLCH we are continuing to re-shape the way Health Visitors work. We have a diverse and inclusive team of Community Nursery Nurses, Community Staff Nurses and Health Visitors who work collaboratively with the Trust Safeguarding team and all our families to support the Healthy Child Programme mandated checks. The aim of our Health Visiting multidisciplinary team is to keep our children and their families safe.

Main duties of the job

  • You will work as part of an integrated team to deliver the healthy child programme
  • You will work in partnership with families to develop and agree tailored health plans to address their parenting and health needs.
  • You will identify, prioritise and implement programmes of care that meets the public health needs of the population.
  • You will provide parenting skills support and information both through coordinating/participating in parenting groups and one to one home visits to families, especially those children and families identified with complex health needs/vulnerabilities
  • You will actively participate in service development activities
  • You will work in collaboration with other health care and service providers to ensure seamless delivery of services for clients and communities
