We are seeking proactive Registered Nurses looking for a new challenge to join our Unplanned Care Team who provide an urgent response to both support and provide unplanned short-term care and to patients in their home environment.

The successful applicants will join a Multidisciplinary team, with a 'can do' attitude, passionate about admission prevention, and team working.

Previous experience of working in the NHS is essential, community experience is desirable, but training will be given. You will be self-motivated with strong organisational skills and able to make decisions under supervision from senior members of the team.

The role requires you to be able to keep accurate clinical records and have good clerical/ administrative skills. You will be a dynamic individual who is able to work with unpredictable caseloads with a variety of medical conditions to assist in avoiding Hospital Admission's wherever possible.

Main duties of the job

The service covers the 4 localities of West Hertfordshire - St Albans and Harpenden, Watford and Three Rivers, Hertsmere and Dacorum. This post offers a great opportunity develop clinical skills and have experience working in the community.

The organisation is supportive of training needs and there is an excellent mandatory training and education programme. You will also receive regular clinical supervision and safeguarding supervision.

This is an exciting post, with the opportunity to help develop the service and make a real difference to patients and a chance to develop both clinical and strategic skills.

The aim of the service is to maintain the safe delivery of unscheduled care and services on behalf of Community nurses, within the local community.

We provide a service 7 days a week 08:00-22:00, working closely with other community services and PCN's to provide high standard holistic care to the population of West Hertfordshire.

  • The post holder will undertake unscheduled community nursing duties involving delivery of nursing services and holistic assessments of patients health needs using a person-centred approach.
