Job summary
Working as a diabetes specialist dietitian within a community diabetes specialist service. Caring for people with Diabetes within the community and working closely with MDT.
The post holder will be an expert dietetic practitioner as a Diabetes Specialist. They will work autonomously without direct supervision, to deliver evidence-based care, education, and information to people with diabetes and their families referred to the Community Diabetes Specialist Service at Wembley Centre for Health and Care.
Main duties of the job
The post holder will demonstrate and understanding and commitment to providing all patients with Diabetes the help they need to identify what is important to them, what they would like to achieve and how they plan to achieve it. Post holder will demonstrate that they have the attitudes and behaviours required to support people to make and sustain lifestyle changes.
The post holder will provide specialist clinics, structured education programs for adults with diabetes and training programs for health care professions.

To take a Dietetic lead within the Community Diabetes Service to provide a specialist clinical Dietetic Diabetes Service, which contributes to the strategic planning and delivery of the multidisciplinary Community Diabetes Service in line with the strategic direction of the Service and Trust objectives.
To be a key member of the multidisciplinary Community Diabetes Service to plan and deliver high quality integrated diabetes care in the community.
To provide specialist assessment and treatment of clients with Type 2 diabetes, liaise with other agencies and health professionals and maintain accurate clinical records.
To be responsible for the implementation, delivery and evaluation of local engagement projects which meets NICE guidelines.
To be responsible for project managing the planning, establishment and continued support of local Diabetes UK support Groups,
To utilise project management skills and work in partnership with service users, community agencies and professionals to plan, develop, implement and evaluate service developments and diabetes prevention interventions based on current evidence based guidelines and policies, which respond to local need, Trust strategic objectives, government initiatives and national policies, e.g. Diabetes NSF, NICE etc.
To lead on the development and implementation of dietetic clinical standards, policies and procedures for the multidisciplinary Community Diabetes Service.
To lead on specific dietetic clinical audit and/or research projects for the multidisciplinary Community Diabetes Service to inform Clinical Governance strategy and service development.
Develop and improve professional expertise via continuous professional development.
