Band 5 Progression Role to Band 6

Are you interested in developing your knowledge and skills in the diabetes field? We are excited to offer the post as a Band 5 development post (competencies to be completed within 12-18 months) to progress to Band 6to join the diabetes team within the Inner London Boroughs.

This is ideal for a dietitian seeking to gain experience in diabetes in a challenging and high paced environment. You will be working with experienced and supportive teams. We are a very friendly and welcoming service where staff wellbeing is a high priority.

We are looking for a motivated, innovative dietitian with excellent communication, organisation and team working skills, working within the diabetes service. You will need to be flexible and have an ability to problem solve and apply clinical reasoning effectively, which are essential as you will be working in a fast-paced and busy environment.

This post offers a wide range of experience in the diabetes field, and it is not necessary to have diabetes experience as you will be supported by colleagues within the diabetes Central London Community Healthcare Trust, to help with your progress.

Main duties of the job

  • To provide diabetes assessment and treatment to clients in a variety of community settings including diabetes clinics, group education programmes, domiciliary visits, and supporting GP practices within the Inner London Boroughs
  • To be responsible to manage their own clinical caseload as an autonomous worker, whilst working to service standards under the supervision of a specialist diabetes dietitian
  • To contribute to the Multi-disciplinary team.
  • To support the management of patients with diabetes and their families
  • To be involved in student dietetic training.
  • To have excellent communication skills, the ability to work well in a team and a positive and flexible approach.
  • To be responsible for inputting clinical activity data accurately.

Job description

Job responsibilities

  • To provide clinical service delivery which is evidence based and conforms to best practice and is supported by research, including comprehensive diabetes assessments, individualized dietary advice and support for all patients to adults in community settings such as GP practices, health centres and client homes
  • To use a range of IT systems to produce data, reports, presentations and literature.
  • To support the service and provide cover where required across CLCH boroughs.
  • To contribute to diabetes guidelines and formulation of other guidelines.
  • To research, develop, produce and maintain evidence based departmental diabetes literature/resources.
  • To work autonomously and manage a busy clinical workload.
  • To provide diabetes service in a variety of community settings. This may include delivering some sessions in the evening and at weekends, when required.
  • To use professional judgement and clinical expertise to independently interpret and analyse complex situations and information to formulate treatment plans according to evidence based practice and modify according to changes in clinical condition.
  • To liaise with the multidisciplinary team to ensure a personalised and effective package of care is delivered to the clients.
  • To be clinically competent to give advice, treatment and dietary counselling to support behavioural change, in adults with complex and diverse healthcare needs across health and social care.
  • To undertake audits and research as part of the role, and to publish and present any relevant data acquired at appropriate meetings/conferences.

  • To use motivation and counselling skills to facilitate acceptance, behaviour change and ongoing compliance with treatment programmes taking into account emotional, physical, social and psychological barriers as well as strong influences on change e.g. depression, language difficulties, impaired cognitive function, resistant attitudes and religion.
  • To initiate and participate in multi-disciplinary/multi-agency collaborative working to promote continuity of care.
  • To use clinical judgement to refer clients to other services and professionals and when appropriate.
  • To provide timely and informative client reports and discharge summaries for GPs, consultants, other Allied Health Professionals, nursing and other staff.
  • To be responsible for effective time management, meeting deadlines and responding to fluctuating workloads and organising, planning and prioritising clinical and non-clinical workload.
  • To be professionally and legally accountable for all aspects of own work and to ensure that own work is within professional standards and clinical guidelines.
