***To support Birmingham Community Healthcare Trust and the city of Birmingham system colleagues and citizens using knowledge of available community services to enable service users' timely community care/support that is responsive to presenting needs thus also preventing hospital, long term/residential care admissions if appropriate. This involves the liaison/close relations with system colleagues such as health/social care/voluntary sector.

Main duties of the job

  • As a senior team administrator, mentor and provide ongoing support to team administrators demonstrating good verbal, written and electronic communication skills.

  • Chair team meetings as required, following up actions, explaining outcomes of activities and interventions.

  • Report any difficulties or problems at an appropriate time to a senior team member.

  • Keep accurate and complete records of activities and communications in a manner that is consistent with relevant legislation, policies and procedures.

  • Manage often complex/non-complex clinical information accurately and confidentially in a manner which is:

  • Prioritise and organise own work and that of supervised staff using own initiative within the policy and procedures and ensure cover arrangements for the referral line are implemented whenever the post holder is not available.
