Job summary
The Integrated Care Systems (ICS) are a new way of planning and organising the delivery of health and care services. They bring together NHS, local government, and third sector bodies to take collective responsibility for the health and wellbeing of the people of Birmingham and Solihull, with the aim of delivering better, high quality, more joined up care for local people and patients within our collective resources.
Together we want to work with community stakeholders and providers across the ICS to make significant changes to the way health and social care are delivered locally to improve the health outcomes for the Black African and Caribbean community. In addition, the 'NHS Long Term Plan (2019)' and 'We are the NHS People Plan 2020/21' explain how systems should be working together to be sustainable, tackle health inequalities and respond to the changing needs of the population.
A commitment to tackle inequalities experienced by the black community is at the heart of the BLACHIR ICS taskforce work. Recent events including the coronavirus pandemic, the Black Lives Matter movement, and economic recession have highlighted the importance of this agenda.
Main duties of the job
The post holder will:
- Work collaboratively within matrix cross-organisational teams, to support the development and delivery of the 39 opportunities for action as highlighted in the BLACHIR report.
- Ensure community, professional, clinical leadership and co-production is central to the delivery of all the BLACHIR Project activities.
- Work with the Black African and Caribbean communities who are most vulnerable on locally based approaches to living well with long-term conditions and reducing risk of ill health (e.g. diabetes, smoking, hypertension, obesity). Support the development of approaches to maintaining good health that are co-produced with communities and culturally humble and safe.
- Keep a helicopter view of the impact, risks and success of workstreams.
- Ensure that all projects are underpinned by effective project planning, communication, and engagement, monitoring and evaluation, risk management and escalation processes.
- Maintain robust records and document logs for reference.
- Maintain good governance around all aspects of programme delivery.
- Work with internal/external key stakeholders to develop and review all elements of project delivery.
- Provide written and verbal reports.
