Due to the promotion of the current post holder we are seeking to appoint a Diabetes Clinical Nurse Specialist for our in-patient services within Adult and Specialist Rehabilitation. The in-patient services operate from a number of sites across the city and the Diabetes Clinical Nurse Specialist will be expected to cover for each site.

The role of the Diabetes Clinical Nurse Specialist will include managerial, operational and specialist clinical responsibilities, providing support, training and advice /Inpatient staff, patients, carers and the wider multi-disciplinary team.

The successful candidate will be a credible clinical leader with the ability to motivate and inspire staff to deliver the highest possible clinical and professional standards. You will be a clinical role model and will be passionate about improvement and patient-centred care. You will possess relevant professional qualifications and will be responsible for maintaining and updating your own knowledge and skills, together with providing evidence in support of this.

Main duties of the job

The Diabetes Clinical Nurse Specialist will have responsibility in line with national and local guidelines to provide advanced assessment of patients needs and advise on appropriate diabetes management. The role will involve empowering patients with the skills and knowledge necessary to attain independence in self-management.

The CNS nurse will undertake duties virtually using digital platforms and in face - face settings and across various Inpatient sites within (BCHC). You will be expected to deliver patient centered advice, support and care for patients with diabetes, their families and carers.

The role of the Diabetes Clinical Nurse Specialist will include managerial, operational and specialist clinical responsibilities, providing support, training and advice /Inpatient staff, patients, carers and the wider multi-disciplinary team.

As a core member of the diabetes service you will have responsibility to provide a quality service and support the development of locality based services to support the community, the primary care networks and ongoing service transformation. The role will focus on those aspects that require greater expertise and experience, whether in management, direct clinical care or the supervision and development of other health care professionals.

Working as a senior nurse you will be responsible for maintaining and updating your own knowledge and skills, together with providing evidence in support of this.
