Main duties of the job

  • To provide clinical care to patients through out-patient clinics, domiciliary visits and MDT reviews. The main responsibilities to include undertaking psychiatric assessments, arranging appropriate investigations, supporting the implementation of treatment plans in collaboration with the MDT.
  • To work autonomously and will be responsible for their own caseload.
  • The completion of certain mandatory assessments such as the HoNOS - LD.
  • To liaise with the MDT and GP regarding any change in treatment & maintaining clear & accurate prescribing records.
  • Compiling a patient's history from a number of sources, and preparing case summaries and discharge summaries
  • Preparing reports for Mental Health Review Tribunals and Managers' Hearings and giving evidence
  • To take part in weekly multidisciplinary team meeting and lead where appropriate
  • To be responsible for relevant documentation follow-up and any necessary discussions/communications with relatives, carers and other disciplines.
