Job summary
The Community Specialist Lipidology Service is seeking an enthusiastic and driven individual to join them to continue a well embedded NHSe funded project until March 2025. This is a secondment or fixed term contract opportunity.
Applications are welcome from people with experience in the management of cardiovascular disease prevention including the management of lipids.
The service aims to bridge the gap between Primary, Community and Secondary Care, and improve outcomes for patients at greatest cardiovascular risk from high levels of Low-Density Lipoprotein (LDL) cholesterol. This role is a blend of virtual patient consultations and in person delivery of education and support in various primary, community and secondary care sites.
Main duties of the job
The successful applicant will continue to support the development of the Lipidology Specialist Nurse role within Community Cardiology. This will include case finding and caseload management. You will assess, plan, implement, and evaluate care as an autonomous practitioner with specialist skills. Where applicable and qualified as an Independent Prescriber, prescribe any drugs within clinical competencies
There will be participation in Lincolnshire's Lipid management steering group working closely with Primary Care, secondary care, community care and the ICB. Education is a key aspect of the role delivering high level updates, training and support to multi disciplinary teams across all services.
Participate in ongoing service review which will shape the service in response to user involvement, professional opinion and audit/survey data along with local and national guidelines/evidence.
Support will be available from the outset and induction will include working with one of our expert specialist lipidology nurses.
