Job summary

An exciting and rewarding opening has arisen within the Lincolnshire Community Cardiology Service. This is an excellent opportunity to make a difference to the cardiac rehabilitation patient pathway in Lincolnshire and aim to increase the cardiac rehabilitation uptake across the county.

We are seeking a proactive, enthusiastic, and innovative individual with experience in the care and management of patients with cardiac conditions to join our dynamic Cardiac Rehabilitation Team in the role of Cardiac Rehabilitation Care Co-ordinator.

Applicants will need to be reliable, flexible, motivated, and autonomous practitioner's that will support the Cardiac Rehabilitation service across the integrated care system ensuring an effective and efficient patient journey. This will be achieved by planning and co-ordinating the episode of care from hospital into the community, promoting safe and effective discharge and improving communication and education.

This role will be predominately based within the hospital setting with the support of our community cardiac rehabilitation team who have a friendly and supportive work ethic. You will have access to regular clinical supervision, MDTs, and daily communication and support.

Main duties of the job

  • To work effectively as part of a team to establish and support robust working relationships across the integrated care system.
  • To work predominantly within the hospital setting at Lincoln County Hospital. This will additionally be a countywide role with an expectation for the individual to travel to other sites across Lincolnshire when required.
  • Provide education to patients and staff to ensure mechanisms are put in place so patients are offered timely intervention and support, creating a process to book appointments with the community cardiac rehabilitation team directly on the ward prior to discharge.
  • Promote the community cardiology service so patients and staff are educated on the cardiac rehab pathway.
