An exciting opportunity for a permanent Patient Safety Performance and Improvement Leadquality improvement and performance lead has arisen within Lincolnshire Community Health Services NHS Trust (LCHS). We are the primary community healthcare provider in Lincolnshire delivering community-based services aimed at supporting people to manage their own health at home and reducing the need for people to go into hospital. As a Trust we care for thousands of patients every day, in partnership with other health and social care services, to deliver joined-up care in community settings. The intention is to provide good out of hospital care and avoid admissions into acute hospitals.
We are looking for a proactive, dedicated and dynamic individual to work as part of our quality team in the role of quality performance and improvement lead.
Main duties of the job
The focus of this post will be to lead and drive delivery of the patient safety, improvement, governance and performance as we move towards PSIRF and LFPSE. Working in partnership with divisional teams and corporate partners to implement patient safety, clinical governance and quality improvement programmes which optimise the clinical care and patient experience and patient involvement.
As a key member of the Trust's quality team you will ensure effective incident reporting and enhanced 'safety through learning' cultures are embedded.
As a Patient Safety Performance and Improvement Lead, you will maintain systems to assure all aspects of patient safety and clinical quality undertaken within the Trust are undertaken in accordance with CQC standards, prevailing legislative and best practice requirements.
You will be a strong team player, an excellent communicator, adaptable, committed, and well-organised and work well under pressure. Experience of project management and service change is also desirable. You must be able to travel around various community locations and work closely with a variety of community, urgent care and specialist services to ensure the quality and safe delivery of services. Data analysis and report writing skills would be an expectation of successful candidates.

To work closely with service leads and clinical staff to implement patient safety, clinical governance and quality improvement programmes to optimise the clinical care and patient experience, responding to a changing context.
Prepare regular reports and present updated complex quality information. Communicate clearly and effectively, orally and in writing, and use welldeveloped listening and influencing skills to work effectively across teams.
Use critical evaluation, analysis of quality and performance data and problemsolving skills to respond to changing priorities to maintain assurance and create opportunities for improvement.
As a critical friend, support deputy divisional leads / service leads to develop, revise, and drive the patient safety agenda and the management of risk.
Use quality improvement methodology to drive patient safety and quality improvement projects, working with colleagues in clinical and non-clinical teams to facilitate implementation of action plans, quality impact assessment and outcomes.
Undertake qualitative and quantitative analysis of all patient safety activities with a focus on risk and incident management data, identifying trends and areas of good practice, concerns and agreed actions to maintain safety and mitigate risk where possible.
To drive the safety through learning agenda embedding the principals of a learning culture.
To support the collation, analysis and evaluation of quality and safety data for assurance and improvement. Contribute to ensuring appropriate service standards and quality markers are incorporated into a quality and safety dashboard.
Facilitate the quality assurance and patient safety frameworks including professional enquiry, investigation, learning and improvements associated with patient outcomes and service development.
Oversee the day to day management of the patient safety resources and tools within the team.
