Within Lincolnshire Community Heath Services we have an exciting opportunity to join our leadership team within Community Hospitals in South Lincolnshire.

The Community Hospital and Community Therapy services in Lincolnshire see thousands of patients every year in inpatient and outpatient locations throughout the county. We promote the Lincolnshire system goals of care closer to home within our frailty and integrated therapy models.

This role will focus on the delivery of care in South Lincolnshire in hospice and ward based settings managing nursing teams. It will involve integration into wider community goals and support integrated care pathways in primary and secondary care. Transformation of services to meet these needs alongside the ability to manage and maintain quality assurance frameworks is key. The role will involve stakeholder development, partnership working with the ICB and primary care colleagues to enable a community optimised approach to care delivery.

Main duties of the job

Candidates will have experience in a leadership role within a planned or unplanned care service. The successful post holder will role model the values of LCHS, be motivated and committed to delivering our clinical strategy, and understand our role as a key partner in delivering health and social care in Lincolnshire. The successful candidate will be responsible for providing assurance against delivery, including performance management, governance, budget management, contract management, professional leadership and business planning. Utilising a range of specialist skills, partnership working and the ability to work at pace is key. As a supportive compassionate leader, you will need to be flexible and resilient to promote and maintain high performing teams.
