Job summary

An exciting opportunity has arisen for a Band 7 Diabetes Specialist Nurse to join the Lincolnshire Countywide Diabetes Team. This role provides an excellent opportunity for a senior nurse who has experience of working with patients with diabetes and has a passion to develop their existing knowledge and skills to shape the current and existing service.

We are open to full time and part time applicants and the geographical area and location of patient caseload will be negotiated with the successful candidate taking into consideration the needs of the Service and location of the individual. We are looking for the individual to be based in the South East or South West of Lincolnshire.

The successful candidate will have extensive experience in Diabetes and ideally be a non-medical prescriber or willingness to work towards this. You will also need excellent interpersonal and communication skills and be able to prioritise tasks. The ability to manage time effectively and work flexibly within the team is essential as is the ability to travel across areas of the county.

Consideration would also be made for an experienced Band 6 clinician who wishes to develop their diabetes skills and knowledge to be become a future senior specialist in diabetes nursing.

Main duties of the job

As a Senior Diabetes Specialist Nurse, you will be responsible for managing a caseload of patients in your area as well as supporting, training and coaching more junior members of staff within your locality to manage their caseloads effectively.

You will have direct line management responsibility for Band 6 staff in your locality, developing a culture of excellence and continual improvement as well as considering the needs of the whole countywide Diabetes service.

You will work with your fellow Senior Diabetes Specialist Nurse colleagues to continually evaluate the performance of the service and develop and implement improvements to ensure the service continually meets the needs of the population.

The Community Diabetes Service is an enthusiastic and forward thinking team delivering high quality specialised care to adults living with diabetes in Lincolnshire. We provide evidence based diabetes care which is able to meet the complex needs of individuals accompanied by the support of a skilled MDT that includes dietitians, GPs, Consultants, Neighbourhood Teams and specialist nurses from other disciplines. As well as an interest in the care of people with diabetes, you will have a willingness to develop the clinical skills and competencies needed to become a specialist and advocate in your field. You will gain experience and exposure to a wide range of clinical skills in relation to diabetes care including recognising frailty, co-morbidities, dementia and management of complex health needs that may impact on diabetes management. We have excellent links with our secondary care colleagues and are continually seeking opportunities to enhance the service specification of the team.
