Job summary

The overall aim of Lincolnshire Community Health Trust is to continue to provide an excellent Urgent Care Service which includes Urgent Care Streaming, Out of Hours, In-Patients and the Clinical Assessment Service, employing a combination of Urgent Care Salaried GPs, Specialty Doctors, Associate Specialists, Advanced Nurse Practitioners and Health Care Support Workers who can work across all the areas, at all base locations, with an element of individual choice in hours and location of work.

We also are becomingly increasingly involved in providing Primary care within the community, helping to support Practices in a variety of ways.

Job description

Job responsibilities

1. To provide, as part of a core team of Urgent & Primary Care Specialty Doctors, Salaried GPs, Practitioners, Trainee Practitioners, Nurses and other support staff, high quality in and out of hours medical services to the community of the Lincolnshire in line with local and national priorities.

2. To provide consultation and appropriate clinical management of patients accessing Urgent Care services in Lincolnshire, via NHS 111, 999 or by self-presenting at Urgent Care centre.

3. Where clinically indicated to provide consultation and appropriate management for patients during OOH who are visited at home or in institutions.

4. If rostered for CAS shifts, to provide efficient and clinically appropriate management by telephone triage consultations of patients contacting the service. Support of the CAS is also required at times of high demand for all Clinicians working within Urgent Care, including Clinicians that are rostered for other duties.

5. To develop and encourage positive working relationships with local GP Practices, housing, social work, voluntary services, drug and alcohol, mental health, dental, secondary care and other appropriate services.

6. To keep contemporaneous, accurate and legible records of all patients encounters and contribute to electronic data recording and audit as directed by LCHS (significant Event analysis will be undertaken in conjunction with the Medical Lead for Urgent Care and the Medical Director of LCHS).

7. To pursue continuing professional and personal development and undertake Appraisal and Revalidation as required by LCHS.

8. To participate in audit and other clinical governance processes.

9. To participate in the supervision, education and mentoring of students, trainees and other professionals as may be required by LCHS.

10. The post holder will be subject to both informal review and formal annual appraisal with particular reference to the Governments Quality Requirements for OOH provision dated January 2005.

11. To assist in the facilitation of and development strategy in the continuing education and upgrading skills of medical and nursing staff.

12. To demonstrate a willingness to undertake additional professional responsibilities at local, regional and national levels.

13. The duties of the post are subject to review from time to time in the light of changing requirements of the service.

14. The post holder may be requested to perform exceptional irregular commitments outside normally rostered duties as are essential for the continuity of care and in occasional emergencies and unforeseen circumstances.

Qualifications

Essential

  • MBBS or equivalent
  • Full registration with the General Medical Council and license to practice
  • Served for a minimum of four years in the registrar or staff grade and/or specialty doctor grade and/or in the clinical and/or senior clinical medical officer grades, at least two of which have been in the appropriate speciality.
  • Exposure to both urgent and non-urgent health care.
  • ILS or higher
  • PILS
  • Safeguarding certificate Level 3 for Children and vulnerable Adults

Desirable

  • Higher qualification, e.g. MRCGP, MRCEM, FRCEM, ALS, PALS
  • Membership or Fellowship examination with the relevant UK Royal College
  • Have completed a paediatric assessment module as part of paediatric advanced practitioner programme (level 4) demonstrating attainment of the required knowledge skills and competence to be able to undertake initial health assessments of 'Looked after Children'.

Experience

Essential

  • Served for a minimum of four years in the registrar or staff grade and/or specialty doctor grade and/or in the clinical and/or senior clinical medical officer grades, at least two of which have been in the appropriate speciality. Equivalent service is also acceptable, with the agreement of the relevant College or Faculty Regional Adviser and the Regional Postgraduate Dean;
  • Completed 10 years medical work (either a continuous period or in aggregate) since obtaining a primary medical qualification which is (or would at the time have been) acceptable by the GMC for full, limited or temporary (but not provisional) registration. Placement on the overseas list will not by itself count towards the qualifying period.
  • A high level of specialist knowledge and expertise with the capacity to work independently within agreed lines of responsibility
  • Participation in a broader role within the trust, for example in education, quality improvement, appraisal or research
  • Exposure to both urgent and non-urgent health care.
  • Experience of patients of all ages.
  • Experience of managing patients with commonly occurring conditions, including diabetes, respiratory conditions, heart related problems, mental health problems, urological and gastrointestinal problems, dermatological, ENT and musculoskeletal problems.
