Adult Community Therapy

Main duties of the job

The Community Therapy teams offer specialist holistic therapy assessments in the patients' own home, care homes and Community Hospitals, aiming to increase independence and prevent hospital admissions, where possible. The service has access to a Transitional Care bed stock for step-up from the community or step-down from hospital. The Therapists assess these patients in a timely manner to facilitate discharge planning and reduce length of stay. Care planning and patient-centred goal planning is a fundamental part of the patient's journey and integration with social care teams, nursing teams, and other stakeholders is essential. The team also has a proactive approach with emphasis on self-management of conditions, which helps to empower the patients and aims to improve management of patients in our service.

We are rated 'Excellent' by the Care Quality Commission, delivered on our financial plans in 2017/18 and are playing an integral part in shaping future health service provision countywide alongside partner health and social care organisations. This includes having an active involvement in Better Together workshops, attending and participating in Neighbourhood Team meetings, and completing integrated work with our nursing teams and other specialist services within LCHS. This provides fantastic opportunities for networking and building on skills and competencies. Together with the Clinical Practice Educators, LCHS is dedicated to supporting learning and development of staff, including the preceptorship programme for newly qualified practitioners and return to practice candidates.

Essential

  • Degree or equivalent postgraduate diploma in Physiotherapy
  • Current Health Professions Council Registration
  • Evidence of relevant postgraduate training
  • Evidence of continuing CPD
  • Basic IT skills

Desirable

  • Member of Chartered Society of Physiotherapy

Previous experience

Essential

  • At least 2 years experience as a physiotherapist
  • Experience in working in the NHS
  • Evidence of leading in areas of team working and/or development

Desirable

  • Experience in multi-disciplinary working

Knowledge/skills/aptitude

Essential

  • Good communicator in a variety of levels and methods

  • Supervision of junior, students and unqualified staff
