Job summary

We are looking for two highly skilled, independent and innovative practitioners to take for the Frailty rehabilitation pathway in Lincolnshire Community Health Services (LCHS) and Lincolnshire as a whole.

One OT and one Physiotherapist will work across two recognised services,Adult Community Therapy and Community Hospital Therapy. This will be to provide clinical leadership and development of practitioners, problem solving and holding their own caseload of complex patients that require specialist Physiotherapy or OT assessment and treatment.

Main duties of the job

Adult Community Therapy and Community Hospital Therapy are not new services in Lincolnshire but the patients they encounter are becoming ever more complex. Meanwhile the pressures on the Health and Social Care system in Lincolnshire grow ever greater. Therefore in order to maximise peoples quality of life and enable them to return to their preferred place of care as soon as possible these roles are necessary.

Successful candidates will:

  • Provide effective clinical leadership in frailty assessment and management.
  • To act as a role model and a resource for the rest of the teams and develop and support individuals.
  • Use a strength based approach to manage and organise a personal, clinical workload offering highly specialist assessment, advice, management and intervention to patients presenting with complex and differential diagnoses.
  • Work with the Professional Leads in the Adult Community Therapy service and Community Hospital Therapy service to develop teams, contribute to clinical developments and support individuals to develop.
  • Work with internal and external systems partners to innovate and develop frailty pathways.
  • To be an acknowledged specialist who develops and shares innovative practice and service delivery models, considering all available models and matching service strands to local need.

Job responsibilities

  • To provide specialist assessment and treatment of specialist caseload for community based adult services across LCHS Adult Community Therapy and Community Hospital Therapy.
  • To provide effective clinical leadership in frailty assessment and management for community based adult services across LCHS Adult Community Therapy and Community Hospital Therapy in accordance with professional standards and codes of conduct, national best practice and locally and nationally agreed standards.
  • To act as a role model to provide specialist advice/support to all members of the primary/social care team on clinical practice issues within the area of falls and frailty.
  • To act as a resource for clinical staff, via mentoring / training, clinical supervision and individual case critical incident analysis, to enable those staff to effectively manage patients in their care who are considered to be frail.
  • To develop and maintain effective communication with all clinical teams, service users and health and social care professionals.
  • To lead and participate in the development of protocols and clinical care pathways, to facilitate best practice within frailty, in line with local and national strategies.
  • The post holder will provide critical thinking and analytical skills in practice, and attainment of knowledge, and use their experience to support practitioners in the development of their own skills and knowledge.
  • Undertake high standards of clinical record keeping including electronic data entry and recording of patient records, report writing and the responsible exercise of professional self-governance in accordance with professional codes of practice and Trust policies and procedures
  • To be an acknowledged specialist who develops and shares innovative practice and service delivery models, considering all available models and matching service strands to local need.
  • To manage and organise a personal, limited, clinical workload offering highly specialist assessment, advice, management and intervention to patients presenting with complex and differential diagnoses resulting in a fall / potential to fall or complex frailty presentation, liaising with other clinicians and providing supervision as indicated
  • To be professionally accountable as an autonomous professional for all aspects of work in relation to caseload and delegation to support staff and students
  • Maintain an awareness of national, organisational and professional developments likely to impact on the service.
  • Contribute to clinical developments within community therapy and ensure that any changes to practice are implemented, monitored and sustained.
  • To provide and implement clinical audits to demonstrate the outcomes of the services.
