Job summary

The adult community services teams provides holistic person centred care that enables individuals to increase independence, improve health outcomes and reduce unnecessary admissions to hospital or long term care. This is achieved through timely interventions, rehabilitation, management of long term and life-limiting conditions and partnership working across the health and social care community. All staff within the service work in collaboration with the patient, their carer and other agencies.Community services teams are required to be flexible, responsive and timely in order to ensure accessibility and equity to all patients to meet the needs of the service in a flexible manner

Main duties of the job

An exciting opportunity has arisen for a self motivated, experienced Diabetes Specialist Nurse (DSN) to be at the forefront of driving up standards in diabetes care within the Community Diabetes Team for Lincolnshire Community Health Services NHS Trust.

The successful applicant must have extensive experience of working autonomously within the field of diabetes and be a non medical prescriber with the ability to work to evidence based guidelines; audit the care given to patients to make improvements in services and show evidence of caseload and personnel management. Be required to deliver educational packages to health care professionals and patients around effective diabetes management.

Have excellent interpersonal and communication skills, be highly motivated and positive in your approach to care delivery to ensure we maintain a high quality service delivery to our patient population.

Must be able to travel across the County to locations not served by public transport.
