The Lincolnshire Community Heart Failure Service is expanding. We have two exciting and rewarding opportunities available for experienced Band 7 Heart Failure Advanced Nurse Practitioners within our community-based Heart Failure Service. We are looking for driven, enthusiastic, and innovative individuals with expertise in the care and management of patients with heart failure to join our dynamic team.
Main duties of the job
The successful applicants will be responsible for managing a caseload of patients with highly complex or unstable heart failure needs in their own homes, bothfaceto face and via video / telephone consultation.Care will be provided at all stages of the trajectory, from diagnosis to end of life. You will work with the patient to formulate individual self-management plans that optimises health and well-being, enabling patients to remain in their own home and prevent inappropriate hospital admission. You will utilise advanced clinical assessment skills to diagnose and treat signs of instability, optimise heart failure therapy andsignpostto other health care professionals where indicated. You will link with secondary and tertiary care providers as required to ensure seamless patient management strategies. You will also be a source of expertise for heart failure care in thelocality, andassist to support development of clinical services that ensure delivery to all quality standards. You will provide on-going specialist education, training and clinical support relating to management of heart failure within the Trust and to partner organisations as appropriate.

The Community Heart Failure Team is engaged in system wide change, having strong supportive networks across the Integrated CareSystemand are committed to transforming the way that heart failure care is delivered - patient focused and closer to home.
The Team has a friendly,supportiveand strong work ethic. "Huddles" are arranged three times per week to discuss clinical cases / clinical supervision with a fortnightly virtual multidisciplinary meeting with cardiologists from secondary care.
This is a countywide role so applicants should expect to travel. Geographical area and location of patient caseload will be negotiated with the successful candidates taking into consideration the needs of the Service and location of the individual
