Job summary
The Lincolnshire Community Heart Failure Service is expanding. An exciting and rewarding opportunity has arisen for three new Heart Failure Specialist Nurse posts in our community based Heart Failure Service.
We are seeking driven, enthusiastic and innovative individuals with an interest in the care and management of patients with heart failure to join our dynamic team.
Main duties of the job
You will be supported to manage a caseload of patients with highly complex or unstable heart failure needs in their homes or other community settings in either face to face, telephone or video consultations.
You will work with patients to formulate individual self management plans, enabling people to remain in their own home and prevent avoidable hospital admissions. You will use advanced clinical assessment skills to diagnose and treat signs of instability, optimise heart failure therapy and sign post to other health care professionals where indicated.
The successful applicant must have access to a vehicle for work purposes. This is a countywide role so applicants should expect to travel. Geographical area and location of patent caseload will be negotiated with the successful candidate taking into consideration the needs of the Service and location of the individual.
About us
