Job summary

An exciting opportunity is available for a dynamic individual to work within Lincolnshire Community Health Services NHS Trusts (LCHS) Cardiac Rehabilitation Service.

We are looking for a reliable, flexible, motivated autonomous practitioner with specific knowledge and experience in cardiovascular disease to provide specialist clinical care using well developed assessment skills to monitor health and symptoms of cardiovascular disease, initiate clinical treatments, provide recovery education and advice, refer appropriately and co-ordinate the patients journey across all parts of the health, social care and voluntary sector systems.

You must have First level registration HCPC registration, Degree level qualification or equivalent experience in cardiac care and either have or be prepared to work towards a non-medical prescriber qualification. You must have excellent interpersonal and communication skills, be highly motivated and positive in your approach to maintain a high quality service to our patient population.

You must be able to travel across the County to locations not served by public transport.

The base of the post holder will be negotiable dependent on the successful candidate

Main duties of the job

Work as an accountable and autonomous cardiac rehabilitation practitioner, managing a caseload of patients with highly complex cardiac rehabilitation needs in their own homes or other community settings, both face to face and via video / telephone consultation.

Work with support of cardiac rehabilitation team leads to improve clinical outcomes for cardiac rehabilitation patients with by working with them to formulate an individual management plan that optimises health and well-being.

The role will involve providing supervision and training as well as providing expert advice to rehabilitation staff within the service with regard to exercise management of cardiac patients.Utilise clinical skills and knowledge required to carry out appropriate assessments and investigations in order to optimise the recovery of cardiac rehabilitation patients, working under the supervision of cardiac rehabilitation team leads. Actively support self-care and informed decision making by providing patients and carers with advice on living with and managing coronary heart disease. Providing education and advice to patient/families to support; health behaviour changes, lifestyle risk factor management, psychosocial health, medical risk management.

Provide on-going specialist education, training and clinical support relating to cardiac rehabilitation within the Trust and to partner organisations as appropriate.
