Due to maternity leave, an exciting opportunity has arisen in Clinical Pharmacy in General Practice Primary Care Networks (PCNs). We are recruiting to a 9 month fixed-term Full Time Band 7 Primary Care Network (PCN) Pharmacist to work Welwyn Garden City PCN https://www.enhertsccg.nhs.uk/primary-care-networks-pcns

In line with the NHS Long Term Plan and the 2019 Five Year GP contract by NHS England, Hertfordshire Community NHS Trust is providing clinical pharmacy services to PCNs in the local area. This is a unique opportunity for pharmacists to experience work in the rapidly expanding area of primary care. This role is part of an exciting programme of transformation to develop a new model of care across a Primary Care Network, whilst retaining the team support, benefits and familiarity of employment in an NHS organisation.

The PCN pharmacist is a key professional in helping patients get the best from their medicines, through clinical medication reviews and medicines optimisation, working in partnership with established primary care clinicians and local stakeholders. As part of a wider team complimenting clinical pharmacists and technicians, these roles will provide support to the GP surgeries and PCN multi-disciplinary teams, the patients they care for and services to achieve improvement in care quality to achieve local and national health initiatives.

Main duties of the job

The Pharmacist will undertake structured medication reviews and patient facing clinics, working in partnership with the patient's GP practice, nurses, care homes, community pharmacists, secondary care and the local authority.

The successful applicant will need to be self-motivated and able to work on their own, whilst also integrating into a well-established team. You will have excellent organisational, interpersonal and communication skills together with a working knowledge of primary care and the challenges facing medicines management in the new era. You will have excellent organisational, interpersonal and communication skills together and encourage a positive culture of medicines optimisation principles to assist the PCN in achieving local and national medicines-related objectives. Experience of working in a clinical setting with a multidisciplinary team is essential. Experience of implementing Medicines Optimisation projects will be ideal. You will need to have a combination of at least two years post registration experience and a clinical diploma or similar experience. The role will also be supported by a full training package, according to needs.
