Job summary
QVM has 21 rehabilitation beds for adults. The majority of our patients come to us from the surrounding acute NHS trusts but we also take admissions from the patients' own home via the Prevention of Admission Team or local GP services for those patients who need monitoring but not Acute Care services.
Patients are referred to us for either a 19-day pathway, complex 28-day pathway or non-weight bearing pathway. QVM is our designated unit for plus size patients and will have a maximum of 2 plus size patients at any one time. At any one time we can also have 4 patients, within our total of 21, who are receiving neuro rehabilitation post stroke and they are on a 42-day pathway. Their therapy rehab is supported by the Early Supported Discharge team for Stroke (ESD).
QVM has a dedicated nursing team and an Advanced Care Practitioner. Our therapy team consists of physios, occupational therapists and rehab assistants and we have access to dieticians, speech and language therapists and clinical psychologists. We have allocated social workers who are based at the unit.
QVM is an approved site for GP trainees. As such we have GP trainees with us on rotational placements
Main duties of the job
