We are looking for skilled registered Nurses to join our dedicated team in providing compassionate and responsive Nursing care to individuals in their own home across East and North Hertfordshire as part of our Overnight Community Nursing Team based at QVM hospital in Old Welwyn.

Main duties of the job
We deliver adult community services across East and North Hertfordshire in a single,integrated model of care, making healthcare more preventative rather than reactive,and to empower local teams to tailor services to the needs of their localcommunities.
We offer a comprehensive induction programme and the opportunity to work in afriendly, dynamic, professional, and caring team.
Successful candidates will need to have an ability to creatively problem solve andwork on their own initiative and will need to demonstrate an understanding ofcurrent professional and community issues. Community nursing experience ispreferred but there will be opportunities for training and development for the rightapplicants.
A full valid driving licence and access to a car to use regularly for business purposesis essential (unless you have a disability as defined by the Equality Act 2010)

We work with a 'prevention of admission' strategy, assessing patients, troubleshooting and deciding what to do to keep the patient at home.
Ensure a team philosophy embraces a holistic approach to patients, carers and relatives in consultation and agreement with the Community Nursing Sister.
Actively promote good health and development and participate in Public Health Initiatives.
Ensure at all times a sound knowledge of drugs, side effects and contra indications and have regard for security and administration of these.
Follow HCT and Community Elder Abuse procedures including provision of reports and attendances at conferences.
At times act as the patients advocate, raising their concerns about services of HCT or other agencies.
Whilst it is not a legal requirement, Hertfordshire Community NHS Trust encourages and supports all staff to be vaccinated as this remains the best line of defence against COVID-19.
