We have an exciting opportunity for a band 5 to join our forward-thinking Discharge Home to Assess team. This post is a development band 5 to 6 post, where we will provide guidance and support to develop your skills and complete your competencies to help you progress to a band 6 therapist.
We have a large community therapy team across a number of localities within East and North Hertfordshire who work with a variety of different patients. We are looking for a physiotherapist or Occupational Therapist to join our forward thinking and innovative team to continue to provide excellent rehabilitation to patients within their home environment. The post holder has the opportunity to join the discharge home to assess service.
Discharge home to assess, assesses patient within the first 72 hours of discharge from hospital and provide up to 21 days rehabilitation.
The therapist will work alongside occupational therapists, therapy assistants, social workers, community nurses, GPs and care staff. Staff meet regularly for professional development and peer support, the post holder will have the opportunity to supervise and delegate tasks to junior staff and offer placements to physiotherapy students
Main duties of the job
It is also essential to hold a full valid driving licence and have access to a car to use regularly for business purposes (unless you have a disability as defined by the Equality Act 2010).
You will be required to complete electronic patient records (training provided).
The position is full time (37.5 hours per week) but in line with our flexible working approach we welcome applications for part time contracts. A seven day working pattern will be required to cover weekends and bank holidays on a rostered basis.
You will initially be employed as a Band 5 at your current salary whilst you are working through your Band 6 competencies as well as demonstrating these competently in practice. Please be aware that the maximum expected time to complete competencies and become a Band 6 - Senior Therapist, will be approximately 18 months after starting your new role. In the event that you are successful in achieving this, then your salary will be reviewed and your pay band confirmed at band 6 pay step 1.
In the unlikely event that you were underperforming against the competencies, an action plan would be put in place to address these competency gaps; you would remain as a Band 5 - Therapist and the opportunity for a Band 6 at this time will not be confirmed. Salary payment until your competencies as band 6 are met will be AfC band 5 scale.
