East and North Referral Hub Administration Lead - Planned Care

Job summary

We are looking for an experienced administrator to take on this exciting role as Planned Care Admin Lead within the Integrated Care Co-ordination Hub (ICC), on a fixed term basis. The post incorporates the management of our existing Planned Care admin team and the Operational admin team.

The team are going through a period of change with a range of digital processes in the pipeline, so you will be highly motivated, forward thinking and enthusiastic in order to deal with the day to day challenges that arise. You will also need to work in conjunction with different stakeholders across the Trust.

Main duties of the job

The post holder will be required to provide comprehensive leadership support to the Planned Care administration team (including operational administrators and designated reception staff), identifying training needs, overseeing and co-ordinating the daily running of the team in conjunction with the community specialist service leads. Whilst being supported by the Business Support Manager, the post holder will be expected to work independently using their own initiative within procedural guidelines, to meet the needs of community specialist services and ensure activity is completed to a high level and within the agreed KPI's. The post holder will work with community specialist service leads to resolve issues, review changes and manage improvement processes.

The post holder will support the ICCC management team in all aspects of business planning and administration, providing reports, escalating issues and identifying areas for team development.

Job responsibilities

The East & North Integrated care Co-ordination Hub manages referrals into a number of HCT community clinical services.

Our team of administrators act as the first point of contact for all patient and professional enquiries received via telephone calls, email and electronic referral system.

The efficient management of incoming referrals is essential in supporting the community based teams to prevent their patients from being admitted to hospital.

The Administrators are supported by admin leads and deputy admin team leads, whose key role is to supervise and co-ordinate the daily running of the team in liaison with the Team Lead

The Integrated Care Co-ordination Centre opening hours are Monday to Sunday 8.00am to 8.00pm, however the planned care team provides services Monday to Friday 8am-6pm. The successful candidate will be required to join the operational rota providing cover on occasional bank holidays

The post holder will be expected to manage a variety of IT systems to ensure sufficient cover for all services the team supports, equipment is ordered in a timely manner and performance is completed to a high standard.
