Job summary
The Referral Hub manages referrals into a number of HCT clinical services. Our team consists of GP's, Nurses, Therapy, pharmacists and Administrators who care for patients across Hertfordshire.
Our team is the first of point of contact for all patient and professionals across East and North Hertfordshire for care co-ordination within Hertfordshire Community Trust. The efficient management of incoming referrals is essential step in the patient care journey, and ensures a timely and appropriate response to the care needs of our local population.
We are currently looking to recruit an enthusiastic, experienced people-centered virtual ward clinical lead. Ideally, you will have experience in leading teams and excellent clinical skills . You will have a proven track record in ensuring a team preform in line with key performance indicators, and working as part of a multidisciplinary team. This role will be responsible for managing and developing the virtual ward service in conjunction with our system partners.
The Referral Hub opening hours are Monday to Sunday 8.00am to 8.00pm, this role, will be required to join the rotational weekend rota.
This role involves some remote working, and some office based duties, with minimal/ no face to face activities.
A full valid driving licence and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010)
Main duties of the job
- Supporting the flow of patient through HCT services.
- Ensuring clinical effectiveness and safety.
- Supporting with complex patient assessments.
- Dealing with incidents.
- Advising on service developments.
- Implementing new pathways.
- Recruitment and selection.
- Building and Maintaining relationships with internal and external stakeholders.
- Supporting the senior management team.
