An exciting opportunity has arisen to join Herefordshire
Community Learning Disability Teams.
You will become part of a small but tenacious and committed team who are
passionate in ensuring that individuals with a learning disability receive
equitable health care alongside managing an effective caseload and maintaining
regular visits for service users and their carers.
The team are
currently involved in some exciting service developments including STOMP,
Dementia pathway, transitions pathways to better meet the needs of individuals
with learning disabilities. We welcome creativity, enthusiasm and ideas in this
team with a go for it attitude to best meet patients health needs.
You will
receive regular individual and group supervision, on-going CPD & training,
and support within this role to help further develop tour
skills and competencies in this specialist field.
We
offer flexible working and full equipment provided for home working. Due to the
rural landscape of the county, being a car driver is essential but there also
are good public transport links to the main base.

Main duties of the job
- Actively manage an effective
caseload, maintaining regular visits for service users.
- Facilitate/teach
effective systems to carers to enable the transfer of skills required for the
management of service users healthcare needs.
- Facilitate Health
Action Plans for each service user on caseload, to enable access to the full
range of primary and secondary care services available.
- Promote and maintain
good working relationships and systems of effective communication through
liaison with service users, carers, and other professionals / agencies.
- Maintain accurate and
up to date clinical records in line with Trust/NMC standards of practice.
- Record activity to meet
the requirements of Patients Information Management System.
- Attend team and
multi-professional meetings as appropriate.
