Job summary

The Glades Neighbourhood Team is a friendly, supportive, and hardworking team who are looking for enthusiastic individuals to join us. We pride ourselves in providing outstanding care to patients in the Wyre Forest in Worcestershire. The team operates from 07:30am to 18:30pm on a seven-day rota including weekends and Bank Holidays, various shift patterns are available. Flexible working opportunities such as compressed hours are available.

This post is for 32 hours over 4 days out of 7 on a rota basis including weekends & Bank Holidays (30 hours over 3 or 4 days out of 7 would also be considered).

We are looking to recruit a Healthcare Assistant who is innovative, dynamic, and keen to join the team in supporting us to deliver high quality care, and to continue our development of integrated care in the community. The role is varied and rewarding.

The Neighbourhood Team consists of Community Nurses, Senior Nurse Practitioners, Advanced Clinical Practitioners, Occupational Therapists, Physiotherapists, Healthcare Assistants and administrators. GP and social care colleagues work closely alongside.

Main duties of the job

Whilst on duty you will either be working alone or alongside colleagues to carry out delegated tasks in line with the Trust core values.

Following appropriate training and competency assessment, you will carry out specific tasks delegated by the qualified Nurse or Therapist including fitting and demonstrating the safe use of equipment; administering prescribed medications in line with Trust policy; supporting with activities of daily living such as promoting independence with personal care or domestic tasks for those patients on the intensive support pathway, support colleagues with moving and handling patients and other allocated tasks such as venepuncture, wound dressings and recording observations.

All clinical documentation is recorded on a system called EMIS Community in line with Trust guidelines and must be contemporaneous. Clear, concise and timely communication is key to your role and this may be required in a verbal or written format to support patient centred care.
