Job summary
We are excited to be recruiting an individual to join the Community Specialist Palliative Care team for North Worcestershire.
The team is made up of two Consultants in Palliative Medicine, a Clinical Psychologist, and a group of nine highly skilled Clinical Nurse Specialists. As a team we support patients, families and Health Care Professionals address unresolved complex issues associated with life limiting illness at the time when this is needed for patients living in the North of Worcestershire. The team is supported by a Senior Medical Secretary and Administration Assistant
We are looking for an enthusiastic and motivated individual who is organised and able to work on their own initiative. You will need to have excellent literacy, communication and word processing skills, as well as experience of diary and email management in outlook.
Previous experience of working with EMIS would be beneficial but not essential as training will be provided.

Main duties of the job
The successful candidate will be the first point of contact to the service. A normal day will most likely see you completing the following tasks;
Be the first point of telephone contact to our patients, families or health professional by taking all telephone queries, highlighting important message to key senior staff and ensuring all messages are appropriately logged.
Following a period of training you will signpost patients/relatives as appropriate
Managing the effective use of the generic email inbox and its correspondence
Registration of referrals
Book ambulance transfers
Request clinical information for patients who have been assessed in other NHS organisations
Contact patients/relatives as required to advise them of changes to appointments
While there will be some flexibility in the role, we advise our patients that we will respond to all calls received before 4pm therefore we would be looking for someone in the main to be covering up until 4:30pm Monday to Friday to help us achieve this service obligation.
