Job summary
Riverside Neighbourhood Team, based in St Johns in Worcester is a friendly, flexible and hardworking team looking to recruit a Senior Community Nurse with a focus on wound care on a full-time, who will be innovative and keen to join the team in supporting us to deliver high quality care, and to continue our development of integrated care in the community.
We have a team of creative health care professionals focused on compassionate care. We work closely with our GPs, developing relationships with other primary care services, utilising skills of our MDT colleagues to manage complex patients ensuring services are responsive and patient centred.
We are a supportive team who are committed to developing our services to provide the most individual care for our patients, offering flexibility, dedicated time for training & clinical supervision, supportive learning & development department to help guide you in your career progression.
You will be expected to work a combination of early (07:30 15:30), mid (8.30 4.30) and late (10:30 18:30) shifts as well as weekends over a 37.5hour week. We are happy to consider flexible working requests.
We are a community-based neighbourhood team providing care to patients in their own home and in Residential homes. We aim to deliver an effective, high-quality service that puts patients at the centre of their own care.
Main duties of the job
An exciting opportunity has become available for an experienced, highly motivated, team player with a range of nursing skills and passion for improving wounds and leg ulcer management the caseload holder will need to have the required skills to assess, treat and plan care to a very high standard that will highlight innovate and support staff to deliver excellent Tissue Viability treatments. With your expertise you will provide the team with training and knowledge to practice high quality wound care. You will monitor and audit your caseload to ensure all holistic assessments have been completed, referrals to wider primary and secondary care conveyed and specialist recommendations actioned with a clear concise plan of care. An essential element to this post is to have excellent written and verbal communication skills with Audit knowledge.

You will be supported by the Band 7 Operational and Clinical Lead with regular supervision.
