Job summary

Pershore and Upton Neighbourhood Team is a friendly and energetic community team who are looking for an enthusiastic individual to come and join us in supporting patients in their own homes.

Pershore and Upton are both small neighbouring towns surrounded by beautiful countryside and we are looking to recruit Nursing Associates to work on a part-time or full-time basis. We would welcome both experienced or newly qualified Nurse Associates who are keen to join the team and enhance their existing skills.

We work closely with our 3 GP surgeries and maintain effective relationships with other primary care services, and together we are an extremely supportive team who are all committed to developing our services to provide the best individual care for our patients.

We offer dedicated time for training and clinical supervision, with a supportive learning & development department to help guide you in your career progression.

You will work flexibly to provide the service delivery need across the local area (the service operates from 07:30 18:30, Monday to Sunday including bank holidays)

Main duties of the job

Your typical working day will include home visits for adults over 18 years needing a range of nursing interventions. These include caring for people reaching the end of their life, all types of wound care; post operative wounds, leg ulcers, pressure ulcers and dressing regimes. See below for the full clinical skills set required for the role, if you do not currently hold these skills this is what you would be supported to achieve whilst in post.

  • Lower limb assessment, inc. doppler assessment
  • Leg ulcer compression therapy
  • PEG tubes and medication
  • Pleurex / Rocket drains
  • Wound assessments
  • Larvae & VAC therapy
  • Diabetes management
  • Medication administration, inc. end of life as per Trust policy.
  • Catheterisation (male, female and suprapubic)
  • Bladder scanning
  • Continence assessments
  • Venepuncture, inc. blood transfusion license
  • Bowel care

You will work from a mobile tablet where you update your records as you visit each patient, the team then meet at base for lunchtime and handover. Back at your base you will be involved in team meetings, care planning and evaluation, and handover meetings. Your input is a valuable part of the teams work. Although a lone worker in the patients home you will be fully supported whilst you develop your new and extended skills.
