Clinician to join our team which provides a responsive assessment and home
treatment service to those experiencing mental health crisis.
HT provides outstanding care to patients and
are looking for individuals who have clinical skills that encompass urgent
mental health and home treatment interventions and are passionate about
providing care to meet the needs of our patients
HT is integral in providing a specialist
service, comprehensive assessment, treatment and care of service users. You
will work collaboratively with service users, carers and professionals offering
interventions on an alternative to hospital admission basis.
We support
early discharge from inpatient wards through intensive, time limited
intervention in the community. HT also supports primary care through screening/assessment
of urgent GP referrals. Positive risk management is vital in this role.
Main duties of the job
To work as a member of the Home Treatment
Team, providing intensive home treatment to patients and support for their
families in accordance with Trust and Home Treatment Team Policies and
Procedures.
To manage a defined caseload of patients
experiencing an acute mental health episode, using evidence based, client
centred, hope and recovery, health promotion and relapse prevention orientated
principles and practice.
To assist in identifying, developing and
providing a multidisciplinary approach within the service in accordance with
the needs of service users and the requirements of the service.
To utilise a range of therapeutic interventions
for adults with acute mental illness.
Working with service users in their own homes
to promote maximum independence whilst experiencing mental health crisis.
To contribute to the planning and
implementation of induction, training and education of students and new staff.
To provide effective supervision, support and
guidance for non-registered workers and any work delegated to them. To participate
in planning of own clinical supervision.

development of the service and participate in the overall development of the
service ensuring the principles of Clinical Governance underpin all
developments
We operate 7 days a week, 8am- 10pm and you can
opt to do long or short days.
