Job summary

Memory Assessment Nurse 30 HOURS PER WEEK We have an exciting opportunity for a Qualified Nurses to join the Memory Assessment Team which is a countywide service. Providing specialist assessment for people who are presenting with signs of a dementia illness. You will need to have a flexible attitude, have good time management and be able to work autonomously as part of a supportive team. The service is open Monday to Friday 9-5 excluding bank holidays. Flexible working patterns will be considered. The team is based in the grounds of the Belmont Abbey which is peaceful rural location, 2 miles from the main city of Hereford. Main duties of the job

  • Manage a caseload of patient ensure they receive a timely assessment, diagnosis and onwards referral
  • Complete full cognitive assessments
  • Write assessment reports to a high standards
  • Effective and timely discharge planning and coordination.
  • Undertake and understand physical health observations and take appropriate action where necessary.
  • Monitor, evaluate and modify treatment in order to measure progress and ensure effectiveness of intervention.
  • Apply a high level of understanding of the effect of dementia and provide advice on management of the condition, healthy lifestyles and well-being.
  • Carry out complex risk assessment.
  • Timely and accurate reporting and escalation of incidents and safeguarding concerns.
  • Attend regular multidisciplinary meetings.
  • Participate in undertaking professional cover/duty.
