We are looking to
recruit a keen, enthusiastic and highly motivated registered nurse/allied
health professional, who is looking for an exciting new chance to utilise their
clinical skills and experience, making a difference to patients living in the community, who require
support with their mental health needs.
As part of the Herefordshire mental health community
transformation, this is an Additional Roles
Reimbursement Scheme role, employed by the H&W Health and Care NHS Trust, but embedded within the HMG PCN team.
The job will involve delivering psychoeducational sessions to support adults
and young people aged fourteen and over to develop long term healthy coping
strategies to empower them to self-manage and stay well.
Working with the
mental health community teams from young adults upwards, this role seeks to
meet the mental health needs of new and existing patients, entering the primary
care pathway, via their GP.
Hereford Medical Group is a PCN formed from a single, large, urban
practice. There are fivesites
across Hereford, though the main site is Station Medical Centre in the heart of
the city.
Working in a flexible and
autonomous environment, you will be embedded within the GP
surgeries and be responsible for the smooth and effective delivery, of this new
and exciting role.

Main duties of the job
Be embedded in the Primary Care Network (PCN) and be a key member
of the Multidisciplinary Team (MDT) working with individuals registered with GP
practices within a specific PCN with mild to moderate and stable severe mental
health needs.
Provide triage, signposting, assessment, and
brief evidence-based treatment (1:1 and/or group interventions) for people
presenting with mental health problems which will include risk assessment and
risk management. Deliver highly specialised advice to the patients and their
caregivers.
Provide clinical advice and support to Primary Care staff in the
local management of patients with common mental health problems.
The
post holder will also be expected to work collaboratively with the key partner
organisations, patients, carers, clinicians and other practitioners within the
mental health multi-disciplinary teams.
The
service operates from 9-5, Monday to Friday and involves a degree of travel,
therefore it is essential applicants can meet the travel requirements of the post.
