Job summary
In Herefordshire and Worcestershire we really believe in our CARES ethos and values, and our ambition is to transform how health & care is delivered across the county. To do this we need to invest in our project and programme management team and so we are looking for talented Project practitioners to champion, deliver and embed service transformation in community stroke services. To get this right you are going to need to be able to take a person-centred approach to project management this is not the right job for you if you rely on project documents to create relationships. Success for this role will be measured through the quality of relationships, professional practice and conversations that you have with your colleagues and people from across our system.
This role will include the opportunity to undertake some tasks from home where appropriate. There will also be a requirement to cover multiple sites. There is an opportunity for the work base to be negotiated at interview.
Main duties of the job
In this brand-new role you will be the key point of contact for the SQuIRE project. You are going to be someone who thrives on two days never being the same; is prepared to get stuck in; and someone who can help shape our stroke service transformation. Your mission is to empower leaders and frontline practitioners to implement transformation in a way that is data driven and sustainable and offers the best health outcomes for local communities. Funding has been achieved for several projects in the Herefordshire and Worcestershire ICS Stroke Rehabilitation pathway. This includes an external consultancy review of the entire pathway for Herefordshire & Worcestershire, a review of rehabilitation delivery to stroke survivors requiring social care input, including loaning of specialist seating to care homes & development of an upper limb rehabilitation pathway in Worcestershire.
