Broadway, Bredon & Inkberrow Neighbourhood team. This post is suitable to
apply for candidates wishing to start on an advanced practice pathway.
We
are seeking an enthusiastic, motivated individual who has a passion for Frailty
and caring for the older adult. You will be working in the community,
supporting patients in their own home environment including nursing and care
homes with both proactive/planned care and urgent response visits.
Throughout
your development you will gain skills in advanced health assessment, complex
history taking and independent prescribing to holistically manage patients,
taking on more accountability as your role and skill set grows. You will be
supported by an existing team of Advanced Clinical Practitioners and Senior
Nurse practitioners in your professional development.

Main duties of the job
As
your role develops you will be assessing patients using advanced health
assessment skills and making a differential diagnosis, when able to, you will
prescribe medications where required using an evidenced based rationale, within
your scope of practice. Some of this may include ordering and interpreting
blood test results, ECGs, and radiology imaging. You will continue to provide
follow up visits where needed and review the patient as required or refer onto
other services if needed.
You will work as part of the wider Neighbourhood Team
(NT), an integrated team consisting of advanced practitioners, district and
community nurses, physiotherapists, occupational therapists and health care and
therapy assistants, supported by our wonderful administration team. The NT
delivers both routine work to meet peoples physical health needs, for those
unable to leave their home, as well as intensive support with rehabilitation
and to prevent hospital admission. You will also provide proactive care to
patients in care and nursing homes with weekly ward rounds/planned reviews and
urgent visits as required.
There will be regular opportunity for clinical
supervision and training and development outside of the advanced practice
modules. Working within the Herefordshire and Worcestershire NHS Trust, you
will also be able to participate in trust wide initiatives, quality improvement
projects and steering groups.
