Job summary
We are looking to recruit a band 6 Senior
Occupational Therapist to work 22.5 hours per week [3 days] over a 7-day period
on a rota basis, within our busy Neighbourhood Team.
You will be responsible for the initial assessment,
planning, implementation of interventions and review of patients across a broad
spectrum of clinical conditions whilst considering the holistic needs of the
patients. You will work collaboratively with the integrated team members and
you will have excellent communication skills as you will also work closely with
other professionals, outside agencies and family members.
The post holder is required to hold a
full driving licence and have use of a vehicle in order to work in a variety of
settings. You may be asked at times to cover other areas within the community
care service delivery unit.

Main duties of the job
Manage a caseload of patients referred to the Neighbourhood Team (Intensive
Support pathway) who have immediate and complex health care needs
Plan and provide rapid and expert professional Occupational
Therapy assessment, treatment and interventions to patients in their own homes,
to avoid inappropriate hospital admission or to facilitate hospital discharge.
Develop and co-ordinate individualized care and rehabilitation
plans in close liaison with the team, the patient and their carers, as well as
all other relevant services.
Coordinate appropriate interdisciplinary, patient centred care
whilst promoting independence.
Provide health care advice and education to team members, external
agencies and family members.
Triage
patients to ensure prioritization of clinical need.
Be responsible for health promotion, teaching and training of
support workers, pre-registration students, patients, relatives and carers.
Provide education, consultation and advice within other care
settings.
