Job summary

An exciting opportunity has arisen for a highly motivated individual to join the learning disability community team. The role involves providing specialist health support and guidance to adults with learning disabilities. The successful candidate will provide clinical support and advice through the effective management of an active caseload. The post is a minimum of 30 hours per week to full time.

The team you would be joining is compassionate and supportive of both each other and the service users they work with.

You will be a flexible nurse with excellent communication skills and a passion for integrated team working. You will have experience of teamwork and be a confident and assertive clinical practitioner.

Main duties of the job

To provide specialist professional nursing intervention using the nursing process to adults with a Learning Disability, focusing on individuals who have complex health care needs.

Work holistically completing new assessments, planning care and risk assessing individuals with a Learning Disability and supporting the assessments of other professionals ensuring these are appropriately documented

Liaising with outside organisations to improve their understanding of Reasonable Adjustments, and to support service users to access mainstream services.

Work autonomously, in line with NMC Code of Conduct and Trust Values

Caseload management for a small proportion of the Adult Learning Disability Community caseload providing nursing interventions and support

Health promotion

Multi-disciplinary team working with OT, Physio, S&LT, Psychiatry and Psychology
