Job summary

An excellent opportunity has arisen for an enthusiastic individual to join our team, providing comprehensive assessments relating to housing, adaptations, and specialist equipment for residents of the Malvern Hills area.

This a permanent post for 37.5 hours, Monday to Friday, with flexible working hours.

We offer a comprehensive induction and continuing professional development training programs to equip you with the skills and knowledge required. We provide regular peer and 1:1 supervision.

Whilst based within a supportive team you will have autonomy to balance and achieve the demands of this busy and varied role.

You will be an excellent communicator, working with clients, carers and other professionals to achieve excellent outcomes for your clients. You will need to be self-motivating have an excellent work ethic, working autonomously, as part of a team.

Main duties of the job

Within an individuals home you will assess their needs and those of their carers relating to reduced independence as a result of disability.

You will identify and with robust clinical reasoning, make recommendation in relation to their housing, specialist equipment, minor and major (DFG) adaptation needs to support independence, safety, and quality of life.

You will facilitate the provision of your recommendations via the appropriate funding and legislative means.

You will be responsible for the recording, planning, and organising of your own workload.

Working with your supervisor you will identify training and continued professional development needs to ensure you have the appropriate skills.

You will participate in a duty system, triaging referrals received by the service, to ensure clients assessment needs are directed and responded to appropriately.

You will contribute to the smooth running of an office, team and to wider service development.
